Here are the statutory disability planning forms which the Legislature changed in 2017. The Medical Power of Attorney form is for use beginning January 1, 2018. All others are for use beginning September 1, 2017.

These forms are used to plan for one's possible future incapacity. These include powers of attorney (for property and for medical decisions), a directive to physicians and family or surrogates (a "living will") and a declaration of guardian.

Forms based on the Texas Uniform Disclaimer of Property Interests Act, effective September 1, 2015.

These forms may be used to address changes to Texas statutes made in 2011.

Use these forms for various probate options, including probating will with independent administration, determination of heirship and small estate affidavit.

Texas Probate Code Section 128A requires executors to give notice to beneficiaries named in the will within 60 days of the date the will is admitted to probate and to file an affidavit or certificate within 90 days after the date the will is probated. These are notice forms, waiver forms and affidavits for use in meeting these requirements.
